Exclusions
- In-office discount plan; not an insurance plan.
- Not valid if you have an insurance policy or in conjunction with other discounts.
- This plan cannot be used for treatment of injuries covered under worker’s compensation.
- This plan cannot be used for services that are referred to outside specialists.
- This plan cannot be used for hospitalization or to cover hospital fees of any kind.
- No discounts on consumables such as Sonicare, Water Pik, Prevident, etc.
Program Guidelines
- Annual membership fee due on day of joining.
- No refunds of fees at any time.
- Discounted payment due when services are rendered.
- Children listed on plan must live in household and be under the age of 18.
- Membership expires one year from joining the program.
- Members are not entitled to refunds for unused appointments or services.
